All students in the Bachelor of Law and Juris Doctor programmes

1st July 2017 to 30 June 2018

 

Insurance coverage details and exclusion list

  • Group Personal Accident Insurance Scheme 
  • Group Outpatient Insurance Scheme 
  • Group Hospitalisation and Surgical Insurance Scheme 

 
The coverage will be subject to the Terms and Conditions of the insurer's Policy.

 
After being matriculated, you will be able to use the Aviva Medical Card at the panel GP clinics to enjoy the Group Outpatient Scheme. An e-version of the medical card can also be found in the ClaimConnect application. Students will be able to retrieve the e-card when they log into the application, and use the e-card at the panel GP clinics.

 
If you lose the Aviva Medical card, please inform Aviva via email at managed_care3@Aviva-asia.com immediately. A replacement fee of S$10, which is payable to Aviva Ltd by cheque only, will be charged for the replacement card.
