Synopsis

This course explores the concept of transition as a key component in the lifespan development of individuals with disabilities. It examines various types of transitions across age, life stages, and systems of care and the supports necessary to navigate them effectively. Some areas of discussion would include: (a) Transitions across key developmental and life stages for individuals with disabilities; (b) Planning and support systems that facilitate smooth transitions; (c) Implementation of transition processes across different service contexts and; (d) The role of inter-agency collaboration and active family and caregiver involvement.
