Synopsis
This course examines the process of preparing the individual with disabilities for transition from childhood into adult life. For individuals with disabilities, the social emotional changes involved in moving from school or college to a successful and independent adult life make the process complicated. This course also examines the planning needed to support transition to higher education and/or employment, and the resources of health and social care, as well as education services that support the transition to living independently and active participation in society.

Learning Outcome
- Assess the functional abilities of adults with disabilities in relation to basic job requirements in preparation for employment.
- Analyse the requirements of job placements to support skills training and work readiness for adults with disabilities.
- Formulate plans that provide opportunities for community inclusion into daily living activities for persons with disabilities.
- Prepare transition plans to higher education or employment using resource mapping.
- Examine procedures for supporting the personal and health care needs of adults with disabilities.
- Propose suitable resources and allied health supports for adults with high support needs.
