EXPERIENCE CARE IN ACTION

Discover How Integrated Care is Delivered Across Communities in Taiwan

As Singapore continues to strengthen community-based care and support ageing in place, leaders increasingly need opportunities to learn from real-world practice. This immersive 4.5-day learning journey takes participants beyond policy frameworks into the communities where integrated care is experienced every day.

Through curated site visits, dialogue with policymakers and practitioners, and guided reflection across Taipei and Yilan, participants will explore how care is coordinated across hospitals, homes, neighbourhoods, and community organisations.

Beyond a study visit, this programme offers practical learning, cross-sector exchange, and actionable insights that participants can bring back to strengthen care innovation in Singapore.

 Why This Learning Journey Matters

Like many countries across Asia, Singapore and Taiwan are navigating rapidly ageing populations, increasing care complexity, and growing expectations for ageing in place.

Over the past decade, Taiwan has strengthened its community-based long-term care system by connecting healthcare providers, social services, local governments, community organisations, and caregivers. This coordinated approach supports older adults to live well in their own homes and communities.

Taiwan's experience offers valuable insights into how integrated, person-centred care can be designed and delivered. Through site visits and dialogue with practitioners and policymakers, this learning journey explores how these approaches may inform Singapore's evolving care landscape.
